Program and Billing Terms

Your program may include non-clinical services furnished by Matter Labs LLC, doing business as Everdawn, and access to clinical services furnished by a Partner Medical Practice, subject to provider availability, legal eligibility, and clinical appropriateness.

Everdawn may administer scheduling, billing support, technology, navigation, and customer support. Licensed providers remain independently responsible for medical judgment and clinical services.

You must be at least 18 years old, complete required identity and eligibility checks, provide accurate information, accept applicable consents, and maintain a valid payment method. Enrollment may be declined where services are unavailable or clinically inappropriate.

State coverage is determined by the applicable Partner Medical Practice and licensed clinician network. A program, clinician, test, pharmacy, or prescription may not be available in every state.

The services included in your program are those displayed and accepted before purchase. Depending on the program, they may include a clinical intake, virtual evaluation, messaging, treatment-plan development, prescription management, or follow-up.

Unless expressly included, program fees do not cover laboratory fees, imaging, medications, supplements, devices, shipping, emergency care, hospital care, specialist services, or services from unaffiliated providers.

The price, billing frequency, initial term, and included services shown at enrollment control. Illustrative pricing or general marketing examples are not binding offers. Taxes and separately elected services may be additional.

You authorize the disclosed merchant or its payment processor to charge the payment method on file for the program and purchases you expressly authorize. You are responsible for keeping payment information current and for charges not prohibited by law.

If you select a recurring program, it renews automatically at the disclosed interval until canceled. Renewal disclosures presented during checkout form part of these terms.

You may cancel using the account, support, or written process provided during enrollment. Cancellation stops future renewal but does not ordinarily refund services already furnished, time reserved, third-party charges incurred, or the current billing period, except where required by law or expressly promised.

You agree to attend scheduled appointments, provide reasonable notice of changes, complete requested forms, disclose relevant health information, follow emergency instructions, and treat staff and providers respectfully. Missed-visit or late-cancellation rules will be disclosed before a charge is imposed.

You remain responsible for seeking emergency and in-person care when needed and for maintaining an ongoing relationship with local health professionals when recommended.

Program enrollment does not guarantee a diagnosis, outcome, laboratory order, supplement, medication, controlled substance, disability certification, or other requested service. Providers may modify or discontinue recommendations based on safety, law, clinical information, or changing circumstances.

Everdawn may suspend or terminate non-clinical program access for nonpayment, fraud, threats, misuse, repeated failure to follow safety requirements, loss of eligibility, unavailability of lawful services, or other material breach. The Partner Medical Practice remains responsible for any clinically required transition or continuity information.

Personal information is handled under the Privacy Policy, Consumer Health Data Privacy Policy, and Notice of Privacy Practices, as applicable. Electronic communications are governed by the Electronic Communications Consent.

We may update program features or fees for future renewal periods after giving notice required by law and the applicable agreement. Material changes do not retroactively alter services already purchased unless you agree or law permits.
