Clinical privacy and medical records
Clinical Privacy Notice
How clinical privacy responsibilities are divided between Everdawn and the independent medical practice that provides your care.
Contents5 sections
1. Everdawn and the treating practice
#Matter Labs LLC operates the Everdawn brand and provides non-clinical administrative and technology services. It is not a medical practice.
Clinical care is furnished by an independent Partner Medical Practice. That practice maintains the clinical record, controls clinical uses and disclosures, and provides the HIPAA Notice of Privacy Practices that applies to your care.
2. Your health-information rights
#Subject to applicable law and the Partner Medical Practice's official notice, you generally may ask to:
- inspect or obtain a copy of your medical record;
- correct information you believe is inaccurate or incomplete;
- request confidential communications or contact at a particular location or in a particular way;
- request limits on certain uses or disclosures;
- request an accounting of certain disclosures;
- obtain a paper copy of the practice's official notice; and
- complain without retaliation if you believe your privacy rights were violated.
The Partner Medical Practice will explain the scope of these rights, response times, permitted fees, and any state-specific protections.
3. Administrative coordination
#Everdawn may receive limited information from the Partner Medical Practice when permitted by law and necessary for scheduling, payment support, customer service, technology operations, or other contracted administrative functions.
When Everdawn acts as a business associate, it must safeguard protected health information under its agreement with the Partner Medical Practice and applicable law.
4. Medical records and clinical requests
#Requests to access, correct, restrict, or obtain an accounting of disclosures from your medical record must be handled by the Partner Medical Practice that maintains the record.
Everdawn will provide or identify the correct clinical privacy contact once the applicable practice is assigned during enrollment.
5. Questions and complaints
#Contact privacy@everdawn.health for help identifying the correct Partner Medical Practice privacy contact. Everdawn and the treating practice prohibit retaliation for making a good-faith privacy complaint.
