Email, SMS, and electronic records
Electronic Communications Consent
Your choices and the risks involved when Everdawn sends agreements, notices, care reminders, email, and text messages electronically.
Contents8 sections
1. Consent to electronic records
#You consent to receive agreements, disclosures, notices, policies, receipts, billing communications, and other records electronically, including through websites, applications, email, SMS, and secure messages. Electronic delivery satisfies a requirement that a communication be provided in writing to the extent permitted by law.
2. Technical requirements
#You need a device with internet access, a current web browser, a valid email address and mobile number, and the ability to view and retain electronic documents. You may print or save documents for your records. Contact us if you require a paper copy.
3. Service and care communications
#Everdawn and the applicable Partner Medical Practice may send messages concerning identity verification, appointments, account activity, billing, requested services, laboratory logistics, medication coordination, care reminders, and other matters related to your relationship with them.
Because ordinary communication channels may display previews or be accessible to others with your device or account, consider who can access your email, telephone, voicemail, and notifications.
4. Text-message terms
#Message frequency varies. Message and data rates may apply. Reply STOP to an eligible SMS program to stop further messages from that program and HELP for assistance. We may send one confirmation after an opt-out request.
Opting out of text messages does not cancel a program or prevent legally required communications through another channel. Some service functionality may be limited if we cannot reach you.
5. Marketing communications
#Where required, promotional email, SMS, automated calls, or similar marketing will be sent only with appropriate consent. Marketing consent is not a condition of purchasing clinical services. You may unsubscribe using the message instructions.
6. No emergency use
#Email, SMS, and portal messages may not be reviewed immediately and must not be used for emergency or urgent needs. Call 911 or seek immediate local care when necessary.
7. Audio and video recording
#This consent does not by itself authorize Everdawn to record a clinical visit. If a visit or call will be recorded, Everdawn or the provider will obtain any separate consent required by applicable law and explain the purpose of the recording.
8. Withdrawal and updates
#You may withdraw consent to electronic delivery by contacting support@everdawn.health. Withdrawal applies prospectively and may require paper delivery, affect the speed of service, or make an online-only feature unavailable.
You are responsible for keeping contact information current. Changes to this consent will be posted with a revised effective date and additional notice where required.
